The sole purpose of the form is to collect information on each player to make communicating and tracking of booster information easier. Completion of this form does give the booster club permission to add your email address to the distribution list used to contact players and parents throughout the year. If you complete this form by July 31, 2023 your player will receive a $5 credit in their player account. NOTE: Booster Club participation is OPTIONAL but players who do not participate through volunteering and contributions are not eligible for some booster provided perks which are made possible through the fundraising efforts of the group, as a team.
